Complete this form 2 weeks prior to the absence. 

Excused absences from band rehearsal include: religious holidays, death in the family, a personal illness which confines you at home.

Unexcused absences include everything else. Examples of unexcused absences may include birthdays, parties, group projects, hair appointments, homework, etc.
